RISK WARNING & WAIVER OF LIABILITY
STREETRULZ FOOTBALL (QLD) — Universal Participation Waiver
One signature covers every adult, child and spectator listed — for all future visits.
1. Provider. This waiver is given in favour of STREET RULZ FOOTBALL PTY LTD (ABN 65 678 776 541), 40 Andrews Road, Tallebudgera QLD 4228 ("StreetRULZ", "we", "us", "our").
2. Activities covered. ALL activities at our facility including (but not limited to): 5-a-side, 7-a-side and all small-sided football; training, coaching, drills, holiday programs, camps, tournaments, birthday parties; use of fields, goals, equipment, clubhouse, café, viewing areas and walkways; spectating inside the perimeter (the "Activities").
3. Risk warning — Civil Liability Act 2003 (Qld) s 5I. Football and active recreation involve obvious risks of falls, collisions, being struck by balls or people; sprains, fractures, concussion, heat illness or other serious injury; and in rare cases, permanent disability or death. These risks cannot be completely eliminated. By entering the facility or participating, you voluntarily accept and assume these risks. This risk warning is issued under section 5I of the Civil Liability Act 2003 (Qld).
4. Your promises. You confirm that: you are over 18 OR the parent/legal guardian of any child listed below; you/the child are physically fit and have disclosed all relevant medical conditions; you/the child will follow all rules, signage and staff instructions, wear correct footwear (no metal studs), shin pads where required, and will not participate under the influence of alcohol or drugs.
5. Medical consent. You authorise StreetRULZ staff to arrange ambulance or medical treatment if reasonably required and accept responsibility for any costs.
6. Release & indemnity. To the fullest extent permitted by law you: (a) release StreetRULZ, its directors, employees and contractors from all liability for personal injury, death or property damage arising from the Activities, except where caused by our negligence that cannot lawfully be excluded; and (b) indemnify StreetRULZ (including legal costs on a full indemnity basis) against any claim arising from your or the child's participation, except where caused by our negligence that cannot lawfully be excluded.
7. Minors (under 18). If signing for a child you: confirm you are their parent or legal guardian with authority to sign; consent to the child participating in all Activities now and in the future; give the release and indemnity in clause 6 on the child's behalf; and agree to supervise or arrange appropriate adult supervision as required.
8. Evergreen. This waiver applies to every visit, booking or activity at StreetRULZ until you revoke it in writing.
9. Acknowledgement. By signing below you confirm: you have read and understand this entire document; you accept its terms freely and voluntarily; and your electronic signature or tick-box has the same legal effect as a handwritten signature.
